What the cycle is, and the small part that's yours.
Every course runs an assessment cycle: pick one skill, measure how students do, adjust teaching, then check whether it worked. Bio 121's focus is CSLO 3 — apply the scientific method, taken straight from the master syllabus.
Form 1 is the plan (filed in March). Form 2 reports what happened — the 76.8% → 83.8% you've been hearing about. Form 3A is the change we made (the Lab 5 intervention). Form 3B verifies it stuck — that's "closing the loop." The same assessment moves through all four, and it rolls up into ICCB program review and HLC accreditation.
You don't fill out the forms — the coordinator and chair do. All that's asked of you:
"You notice, you plan, you teach, you measure — and it fits on a page."

How many students are currently enrolled in your section(s)?
This helps us track participation rates. You'll only be asked once.
Auto-aggregated from student quiz submissions
Your responses feed Form 3A directly — no separate paperwork. Same 8-item instrument, unchanged across the cycle. The item data above is your context.
